Important Note for Reserved Packages
These 25 packages which will reflect in the package list of Private Hospitals
butreferral slip (Government Hospital will give to patient) is mandatorily while
submitting Pre-Authorization in the online pre-authorization form
If any beneficiary patient visits empaneled private hospital for treatment/procedure which is
reserved in the Government Hospital without carrying a referral slip from the Government Hospital,
then such patient shall not be eligible for treatment in the empaneled private hospital under the scheme
